The listing's Name field arrived pre-filled with mcp.contextaco.com — the server URL, not the product.
It looked deliberate, so it survived several passes through the form and was only caught on the final review screen. It would have shipped as the connector's display name in the directory, and it very likely seeds the permanent slug.
What made it look right: every other field on that step was empty, so a populated one read as "already handled".
What was actually wrong: it is the only field on the listing whose default is derived from a different step's answer, and a hostname is a plausible-looking name. Nothing flags it.
The general lesson is stronger than the specific one: on the final review screen, read the fields you did not type. Those are the ones nobody has checked.
